5.3.6 Looked After Reviews: Guidance for Social Workers

SCOPE OF THIS CHAPTER

This guidance should be read in conjunction with Looked After Reviews Procedure. It is designed as a checklist of steps that Social Workers should follow for arranging initial* and ongoing Looked After Reviews.

*An initial Looked After Review must occur with 28 days of Becoming Looked After (BLA)


Step 1

REFERRAL AND ASSESSMENT PROCEDURE

As soon as a young person or child has BLA the allocated Social Worker or Duty Social Worker must complete Step 1 of the  ‘Notification by Referral and Assessment to Care Planning of a child  Becoming Looked After BLA form.

In addition the procedure detailing the arrangements for transfer from R&A to Family Support and Intervention Service should be followed.

FAMILY SUPPORT & INFORMATION TEAM PROCEDURE

  • If a child BLA in FSI the Social Worker should immediately notify care planning business support either by email to Business Support LAC or in person.

Step 2

As the review must take place within 28 days of the BLA date.

  • BSO’s will offer the Social Worker dates within timescales.
  • BSO will look at the availability of the Social Worker and match this with an Independent Reviewing Officer who is available at the same day and time.
  • The venue will be agreed with the Social Worker – ideally the review should take place in the placement.
  • This information will be confirmed by email to the Social Worker letter titled forthcoming LAC review, Independent Reviewing Officer and Team Manager, reminding you of the documentation required for the review.  Attached to email letter will be the monitoring sheet.
  • Section 2 will need to be completed by the Social Worker and Team Manager signed and bought to the review.
  • PLEASE NOTE ARRANGED DATES CANNOT BE CHANGED.

Step 3

In conjunction with the above, a PL2 forms must be completed.  This provides Care Planning with an electronic notification of a child BLA.  As this information may take several  days to filter through we ask that you follow steps 1 to 2  above and go along to our Business Support Officers to notify them immediately a child BLA.

Step 4

  • Prior  to the review please ensure you provide all the necessary documentation for the review and contact the IRO to discuss the care plan and finalise arrangements.
  • Documentation required for the review;

- One week prior to the review please ensure you send the reviewing officer the following documents:

  • Review of Arrangements form, fully completed up to and including Section 22.
  • Copy of current Care Plan/Pathway Plan Summary.
  • All completed consultation documents (please note you are responsible for sending these to the child, Carers and family members as appropriate).
  • Summary of work from the latest ‘action and assessment record’.
  • Copy of chronology.

Please either send or bring with you to the review any other relevant documents or reports for example:

  • Latest health assessment report;
  • Personal Education Plan;
  • Placement Report;
  • Placement with parent’s regulation report;
  • Pre-birth assessment;
  • Risk assessment;
  • CAMSs report;
  • Summary of core assessment;
  • Genogram.
  • It is your responsibility to send invites to all review participants and arrange  an interpreter if required.

Any questions please come and ask our Business Support Officers, LAC Planning Managers or any IRO.

The following information details the process Social Workers and team Managers should follow to arrange an Ongoing LAC review:-

Second review will be within three months of the initial.

Third and consecutive reviews are held at no more than six monthly intervals

The date for the second / third / next review will be set by the reviewing officer at the end of each review.  The IRO is responsible for entering this data onto ICS and the details will be contained at the end of the decisions of the review.

4 weeks prior to the scheduled review Care Planning Business Support will send to the Social Worker and their team Manager the letter titled ‘forthcoming LAC review’. This letter acts as a reminder of the date, time, venue of the review and the documentation required for the review.  Attached to the email letter will be the monitoring sheet.

  • Section 2 will need to be completed by the Social Worker and Team Manager signed and bought to the review.
  • PLEASE NOTE ARRANGED DATES CANNOT BE CHANGED.

It is the Social Worker’s responsibility one week prior to the review to ensure they send to the independent reviewing officer the following documents:

  • Review of Arrangements form, fully completed up to and including Section 22.
  • Copy of current Care Plan/Pathway Plan Summary.
  • All completed consultation documents (please note you are responsible for sending these to the child, Carers and family members as appropriate).
  • Summary of work from the latest ‘action and assessment record’.
  • Copy of Chronology
  • Copy of Permanency Plan this will have been drawn up at the ‘Permanency Planning Meeting’ held before the second review.  (cross reference to permanency planning section)

Social Workers either need to send or bring with them to the review any other relevant documents or reports for example:

  • Latest health assessment report;
  • Personal Education Plan;
  • Placement Report;
  • Placement with Parent’s regulation report;
  • Pre-birth assessment;
  • Risk assessment;
  • CAMH’s report;
  • Summary of core assessment.
  • Genogram

The monitoring form is required for every review.  The Independent Reviewing Officer will complete Section 1.  Section 2 is to be completed and signed by you and your team Manager and brought to the review.

The record of the review and decisions will be distributed by the Care Planning Team with the exception of those for children and young people who are the responsibility of the Adoption Service.  The Adoption Service are responsible for the distribution of review records and decisions for these children and young people.
