1.2.7 Referral of Cases to other Agencies |
Contents
1. Introduction
The Lewisham Safeguarding Board has agreed that the main referral tool that will be used throughout the London Borough of Lewisham will be the Common Assessment Framework (CAF). However, for Children’s Social care the Department for Education requires us to complete Initial and Core Assessments rather than CAFs. It has therefore been agreed that when we refer to other agencies we should send copies of our Initial and Core Assessments to provide the other agencies with the appropriate information that they will require in order to carry out their work.
2. Information Required for Other Agencies
Permission must be obtained from the parents/carer of the child that the information contained within the assessment can be shared with other agencies.
A copy of the assessment and care plan should then be sent with a covering letter containing any additional information that the agency may require and to make clear what we are asking for in the referral.
A copy of this letter must be added to the case notes on ICS.
This letter should ask for feedback on what has happened with the referral from the new agency.
The feedback should be entered on ICS as a case note if the case is still open or as a contact if the case is closed. A new referral should not be created on ICS unless there is a clear role for CSC.
